1. General information

People who  like to adopt a child from other country, for example from Ukraine,tobegin their adoption journey in different ways. Some families contact to American's adoption agencies , take their service . Some families to do an adoption process independently and use a service of  Ukrainian adoption agency Adoption-Ukraine (the cheaper and faster variant).

First of all family must to connect with Ukrainian Consulate in your country and to start adoption process. The Ukrainian Consulate tell you about necessary documents, pleasecall, don't hesitate!

After you have received the list of  necessary documents from Ukrainian Consulate,you must to do these documents and send them to Ukrainian Embassy or as variant you can to send documents to Centre of adoption in Kiev with  special fast post service (FEDEXor DHL). And to add into a packet of documents a special warrant in the name of our agency's lawyer, he will be your representative here in Ukrainian institutions - the Centre of adoption , children homes, orphanages,ministries, court and other serious state's institutions.

If child liked you, we can to continue an adoption process in court. After a positive court decision you must wait 30 days . Citizens of the United States, who have expressed an interest in adopting a child whois a citizen of Ukraine, should make a written declaration to the Center for ChildAdoption of the Ministry of Education of Ukraine (address: 252032 Kyiv, 27 TarasShevchenko Street, telephone 216-27-21) regarding their placement on a list of candidates for adoption and the right to visit appropriate governmental agencies for the selection,visitation, and adoption processes involving such children.

Such declaration should be accompanied by the following documents:

1. Declaration executed by the competent US authorities which confirm the candidate's capacity to be registered, including his status as a homeowner or renter of an acceptable residence, biographical information, familial status - whether married, for how long, how many children and their ages, all other relevant personal information in the event thatthe declaration is made by non-governmental authorities, a copy of the authorization by competent US governmental authorities permitting such fulfillment of this requirement forthe adoption should be attached.

2. Permit of the competent US governmental authorities where the candidate resides allowing the child to be adopted and to enter for legal residence within its jurisdiction.

3. Document regarding receipts, earnings, income (bank statement showing the annual income of the family).

4. Medical Certificate regarding the health of each candidate being registered.

5. Copy of Marriage Certificate (provided those candidates making the declaration are married).

6. Copy of passport or other document proving the identity of the candidate being registered.

7. Declaration by the competent law enforcement authorities for each candidate being registered stating that no criminal activity has ever been engaged in by such candidate.

8. Guarantee of those adopting at the time of the adoption of the child that registration of the child will be made within one month at the Ukrainian Consular Officesin the area in which they reside and updated information will be provided each year regarding the upbringing of the child, it being understood that the candidates guarantee the right of the Ukrainian Consular Officers to visit the child and that the child shallremain a citizen of Ukraine until his/her eighteenth birthday.

The above mentioned documents should be notarized by the Notary Public, County Clerk,Secretary of State and must be "authenticated" by the United States Departmentof State and by the Consular Office of the Ukrainian Embassy. For details see Section"Document Authentication and Legalization Procedures & Fees".

The documents should be translated into the Ukrainian language. The translation shouldbe validated by written approval in the Ukrainian Consular Offices or in the appropriate offices on the territory of Ukraine.

The Center examines the adoption documents presented by the candidates, places these candidates on a waiting list, gives them necessary information regarding the children available for adoption, including all information which would be necessary for adoptionand then gives detailed information regarding where the candidates should present themselves in order to meet with the children who can be adopted.

For the decision of questions regarding the possibility of adoption of certain childrenby foreign citizens, written notice should be given to the appropriate place finalize ofeducation in the city where the child resides.For the resolution of questions regardingthe adoption of a child who is a citizen of Ukraine, foreign citizens should address theCenter for all permits necessary to finalize the adoption.At the time foreign citizensseek to finalize their application for the adoption of a specific child, the Centerexamines all necessary materials and gives written permission for the processing of the adoption or, at that time, a written denial in detailed form.Adoption of the child, who isa citizen of Ukraine, for foreign citizens is effected by the decision of the city(region) court for granting adoption orders.No payments are to be made by candidates forthe adoption of a child except those related to covering incidental office expenses.Noperson acting in an introductory or intermediary capacity is allowed to accept any kind of payment for having facilitated the adoption of a child.In the name of and in the interestof citizens who request the adoption of a child, other individuals may act by specific written power - of - attorney for the purpose of effecting the correct legal transfer ofthe child except in the following capacities:

Candidates for the adoption prior to a decision being made are personally obligated:to familiarize themselves with the documents related to adoption of the child, including oversight by the state of the child's health: to meet and establish contact with the child.

1. Attendance of the candidates seeking to adopt at the time of examination of the adoption papers in court is mandatory.         2. The candidates are personally obligated: toaccept the decision of the court regarding the adoption to take the child from hisresidence by using as proof a copy of the court's decision regarding adoption.

The enabling acts of Ukraine, which decide issues of adoption are:

Civil Code of Ukraine.  Civil Procedure Code of Ukraine. Code for Marriage and Family in Ukraine.

Order giving children, who are citizens of Ukraine, the right to be adopted by Ukrainian citizens and foreign citizens and the stated control verification regarding their living with the adoptive family (Passed by the Cabinet of Ministers of Ukraine,effective from July 20,1996, N 775).

 On January 28, 1996, and effective April 1, 1996, the moratorium on adoption of children in Ukraine was lifted by the Ukrainian parliament .The matters of international adoption in Ukraine are adjusted by such legislative acts as follow:

  3. Code about a Marriage and Family of Ukraine (the Code);
  4. "The Order of transfer of children, who are the citizens of Ukraine, on adoption to the citizens of Ukraine and foreign citizens and realization of the control behind conditions of their residing in new families", authorized by the Decision of the Cabinet of Ministers of Ukraine from July 20, 1996, # 775

The adoption of children will be carried out under the application ofperson wishing to adopt the child in the judicial order. The presence of the prospective adoptive parents at the Court is obligatory (Clause 102 of the Code).

The registration of the foreign citizens, who wish to adopt children, is conducted by the Center on adoption of children at the Ministry of education of Ukraineextremely (Clause 102-2 of the Code).

THERE IS NO LEGAL PROCEDURE FOR ADOPTING THROUGH GUARDIANSHIP IN UKRAINE.

Under US law, prospective American parents wishing to adopt a child in a foreign country must, in all cases, initiate the process with the Immigration andNaturalization Service of the United States (INS) by filing form I-600A, Application for Advanced Processing of Orphan Petition. This petition must be filed at the appropriate INSoffice in the United States with jurisdiction over the adoptive parents' place of residence.

 

REGISTRATION WITH ADOPTION CENTER

Citizens of the United States wishing to adopt a child from Ukraine must register with the Adoption Center to begin adoption proceedings in Ukraine. The AdoptionCenter is operated as part of the Ministry of Education and is the ONLY legal Ukrainian authority that maintains a database of children available and qualified for both domesticand international adoptions. The Adoption Center is involved in international adoptionsfrom the initial to the final stage, e.g. from the moment prospective parents apply for registration to the point when, once a child has been identified, the case is forwarded to a court for adjudication.

Please keep in mind that translators or interpreters are not available on the staff of the Adoption Center, so callers or visitors have to be prepared to speakUkrainian, or have their own interpreters.

The intermediary commercial activity concerning adoption of children,transfer them under guardianship or on education in families of the citizens of Ukraine or citizens of other states is forbidden (Clause 102-3 of the Code), and there will be NO fees except those for court filing, notaries and translation and similar services.


ADOPTION CENTER DATABASE

Citizens of foreign countries may adopt only those Ukrainian children that

Orphanages are responsible for providing full and complete information on children available for adoption to the Adoption Center database in a timely fashion(within 1 week). The local district office of the Ministry of Education creates a file for each child available and qualified for adoption and prepares an application that contains basic information about the child and his/her photo. They retain the application at their office for one month, during which they encourage Ukrainian families to adopt the child.After being kept at the district office for 1 (one) month, the application is forwarded to the regional (oblast') office of the Ministry of Education. This office has 1 (one) month to find a Ukrainian adoptive family for the child.

Note: Under Ukrainian law, disclosure of information on children available for adoption to other organizations or private citizens by Ukrainian officials is forbidden.

If the child still has not been adopted by Ukrainian citizens, his/herapplication is further forwarded to the Adoption Center to be input into its database.Once a child's application reaches the Adoption Center, the Center has 12 (twelve) months to find a Ukrainian family for the child. If the legal status of the child changes, the guardian authority will have to notify the Adoption Center of this change within 7 (seven)days.

Children who, in accordance with list of diseases of the Ministry of Public Health Protection, are determined to be unhealthy are not subject to the'one-plus-one-plus-twelve month' waiting period and will be available for international adoptions immediately.


DOCUMENTS REQUIRED FOR REGISTRATION WITH THE ADOPTION CENTER

Ukrainian law states that foreign citizens wishing to adopt Ukrainian children must submit a petition to the Adoption Center, requesting to be registered as prospective adoptive parents and to be permitted to visit orphanages in order to select,meet and establish contact with an orphan.

The following documents must be part of the petition:

1) Home Study, an affidavit issued by a competent authority in the adoptive parents' country, attesting to his/her eligibility,specifying his/her housing and living conditions, containing curriculum vitae, and other information; if this affidavit is issued by a non governmental entity, a copy of the license authorizing this entity to conduct adoption procedures shall be appended.
Comment: Statement obtained from a social worker agency.

2) Entrance and permanent residence permit for the adopted child, issued by the competent authority in the adoptive parents' country.
Comment: Forms I-171 or I-797 (INS approval forms) will serve in place of this document.

3) Proof of income (bank statement on the parent's family yearly incomes).
Comment: W-2 form or tax returns and a statement from your bank certifying that you have a bank account there.

4) Bill of health issued in the nameof each of the adoptive parents.
Comment: Not just a general statement that you are healthy, but a declaration that you are specifically not suffering from any kind of psychic, communicable, internal, skin or venereal diseases as well as that you are not drug addict; also, AIDS and syphilis tests must be included.

5) Copy of the marriage certificate (if applicants are a married couple)

6) "No criminal record" statement supplied by a competent authority for each adoptive parent, attesting to his/her having no criminal record and included Copy of the passport or other identification papers of prospective adoptive parents.
Comment: statement obtained from local sheriff / police office.

7) Adoptive parents' commitment , if granted the adoption, to have the child registered with Ukraine's consular office in their home country within one month; to supply information (at least once a year) about an adopted child's living conditions and educational process to the Ukrainian consular office, to arrange for consular officers to keep in touch with the adopted child, and to retain the child's Ukrainian citizenship until 18 years of age.

Please note that ALL seven documents from the list must be independent documents.

All documents must be properly legalized at the Consulate General of Ukrainein New York or Chicago, or Embassy of Ukraine in the USA (Washington,DC). Documents will remain valid for one year. Once the documents are legalized, theymust be translated into Ukrainian and notarized. The translation should be validated by written approval in the appropriate Ukrainian Embassy or Consulates or in the offices ofthe government notary in the territory of Ukraine.

The Adoption Center must, within ten days, process the documents submitted by adoptive parents and enter them into the database. Once parents identify the sex and the age of the child they wish to adopt, the Adoption Center shows them applications of orphans available for adoption within this age group. The Center will then issue a letter of referral to allow the prospective parents to visit orphanages and to meet, select and establish contact with the children. Along with a letter of referral, adoptive parents will be given their documents, bound, numbered, sealed, and signed by an official in charge of the Adoption Center, with a separate sheet specifying the number of pages andthe prospective parents' registration file code.

The receipt of any type of payment or favor for the granting of the adoption is forbidden.

No person acting in an introductory or intermediary capacity is allowed to accept any kind of payment for having facilitated the adoption of a child.

In the name of and in the interest of citizens who request the adoption ofa child, other individuals may act by specific written power-of-attorney the purpose of effecting the correct legal transfer of the child except in the following capacities:

GROUNDS FOR DENIAL TO REGISTER

No standards have been established by law for approval or denial of prospective parents' applications to register. Eligibility is determined through a personal interview at the Adoption Center.

However, violation of adoption procedures and regulations (e.g. visiting an orphanage without the Center's permission, meeting and selecting children for adoption prior to registration, not arriving  for an interview, etc.) can be considered as grounds for denial to be registered with the Adoption Center. In case of registration denial, prospective parents must be notified of the denial in writing and all the documents they submit should be returned to them.


MEETING A CHILD

Once the Adoption Center issues permission for prospective parents to visit orphanages, parents may go there and meet with a child(ren), check medical records and establish personal contact with him/her. When a child is selected for adoption, the Adoption Center then removes the child's name from the database of orphans available for adoption.


COURT HEARING

It is important to mention that, in compliance with recent changes and amendments to the Family and Marriage Code of Ukraine, the power to approve or deny an adoption is solely with an individual judge. The judge's decision, in turn, will be based on a review of various documents of each individual adoption case during the court hearing, where adoptive parents have to be present. The law states that adoptive parents must attend the hearing. The Adoption Center has stated that this requirement will be strictly enforced following the recent resolution of the Supreme Court of Ukraine. In cases where one of the parents cannot be present at the hearing due to a compelling reason(e.g. major surgery, disability etc.), a judge may permit one parent to provide a power of attorney to the other parent.

The packet of documents for the adoption case is presented to the judge after being carefully examined by the Adoption Center. As a general rule, the judge's decision is announced and issued the day of the hearing. However, it does not take effect for 30 calendar days, within which it can be appealed.

Once the decision takes effect, the new parents are granted parental rights and legal responsibility for the child.


OBTAINING A TRAVEL DOCUMENT

RAGS (Ukrainian office of vital records) will issue a post adoption certificate of birth for an adopted child based on the final court decree and the original (pre-adoption) birth certificate only. Please remember that you will not be given the pre-adoption birth certificate back. Therefore, please make sure to that you make a copy before handing it over to RAGS authorities.

Once you obtain the post-adoption birth certificate, you may apply for a travel document (passport) for the child at the local VVIR (Office of Visas andRegistrations). You will be required to present the post-adoption certificate of birthand the final court decree.

Rules and Regulations for Authentication of Documents promulgated by the United States Department of State

Quite often, American citizens and business persons require that certain documents be "legalized" so that they can be accepted in Ukraine. The procedures to be followed depend on the nature of the documents and are similar to the requirements of all other governments throughout the world.

Notarization

If the document is in the Ukrainian language and merely requires notarization, it may be signed by the signatory person before Ukrainian ConsularOfficer at the Consular Office who will there after notarize it. The fees for such notarization are $40 dollars per document. This procedure is known as the"notarization" process and is substantially similar to the procedures followed here in the United States.

Authentication by the US Department of State

All other documents which are issued by the US authorities, or which are business documents or documents which have been previously notarized in accordance with the procedures of the various states must be "authenticated" in accordance with the Rules and Regulations for Authentication of Documents promulgated by the United States Department of State. These rules are set forth at the end of this document.

Authentication by the Ukrainian Consular Office

 

After a document has been authenticated by the US Department of State, it may then be submitted to the Ukrainian Consular Office for "authentication."

  • The Consular Office legalizes only those documents, which are duly authenticated by the relevant US authorities as outlined above.
  •  

  • Every document such as Power of Attorney submitted to the Consular Office for legalization must be accompanied by one clear photocopy of each page.
  • The Consular Office accepts processing fees only in the form of money orders or company check.
  • The fees for legalization are:
  • * $60 for legalization of one document to be completed within nine business days notcounting the business day of its receipt by the Consular Office.

    * $120 if the document deals with ownership of apartments or other "realty" etc. or document that is necessary for the registration of the appointment of an agent for foreign juridical persons (legal entities) or a joint venture involving the participation of a foreign juridical person or any document that relates to any kind of business activity with Ukraine.

    If applying by mail, self-addressed, prepaid express envelope is required. ForFedEx Account number is required. Credit Card number is not acceptable. When there is noenclosed prepaid,self-addressed return envelope or a courier service's air bill, thedocument can, upon its owner or other submitter notarized request, be kept at the Consular Office without charge for personal pick up.

    Expedited Service

    At the specific request of a submitting party, documents will be legalized on the dayof their receipt by the Consular Office or on the next business day (up to 3 documents).

    For the expedited service the consular fees are doubled.

    Please, also be advised that documents received on Saturday, Sunday or official Ukrainian or American holidays are considered received on the next business day.

    AUTHENTICATION SERVICES

    The Department of State is responsible for providing authentication services to U.S. citizens and foreign nationals on all documents that will be used overseas. Documents include: company by laws, powers of attorney, trademarks, diplomas, transcripts, invitations, letters related to degrees, distributorship agreements, statements of non-marital status, articles of incorporation, good standing certificates, certificates of merger, home studies, job certifications, letters of reference, deeds of assignments, patent applications, etc.

    The Authentication Office (of the Department of State) is responsible for signing and issuing certificates under the Seal of the United States. It also ensures that requested information will serve in the interest of justice and is not contrary to U.S. policy.


    REQUIREMENTS

  • Signed before a notary public
  • Certified by the clerk o f court of the county in which the notary is commissioned* and
  • Certified by the Secretary of State of the State in which the documents is executed.
  • *Item 2 may be omitted if the authority in item 3 will certify directly to the notary.


    STATE AND LOCAL DOCUMENTS

  • Certified originals.
  • Certified by the Secretary of State in which the documents are recorded. The Secretary of State should be requested to certify to the officials signing the document under the impressed Seal of the State.

  • Must be certified under the official Seal of the United States Department of Agriculture on the State in which the document originates.

    For all documents being authenticated, the following must be complied with:

  • All seals and signatures must be originals.
  • All dates must follow in chronological order on the notary, clerk of court, and Secretary of State certifications.
  • All documents in a foreign language must be accompanied with a certified (notarized) English Translation.
  • If a copy of document is used, it must include the statement that it is a true and accurate copy.

    MAIL-IN

    Authentication takes about 5 days from the receipt to the return of documents received by mail. Include a daytime telephone number, the name and country where the document will be used, and a  self-addressed stamped envelope to return documents.

    The Authentication Office will forward documents directly to one Embassy or Consulate in the United States if provided with a transmittal letter, the fee, and a pre-addressed, stamped envelope for the Embassy or Consulate to return the document to the customer.

    The authentication fee is $5 per document. A personal/company check or money order made payable to the United States Department of State must be submitted with the document. Master Card and Visa are accepted.

    Documents drawn up in the USA in the English language may be accepted in Ukraine with the provision of a certified translation of such a document into the Ukrainian language. The translation should be validated by written approval in the appropriate Ukrainian Embassy or Consulates or in the offices of the government notary in the territory of Ukraine.


    Preparation of translation

    If a translation into the other language is done at the same time as a notarial action (certification of a transaction, certification of the faithfulness of a copy of a document, etc.), the text of the translation is placed on the same sheet as the original: on one page, divided by a vertical line so that the original textis placed on the left side and the translation on the right.

    The translation must be made from the full text of the document to be translated and must be completed by signatures.

    In all other cases, the translation, placed on a sheet (sheets)separate from the original, is attached to the original document (sewn), authenticated by the signature of the consul and the official stamp of the consular office.

    When signing the translation, the translator indicates his authority and certifies that he is competent in the English and Ukrainian languages. The validating endorsement is placed under the texts of the document and its translation.


    Certification of translation

    In accordance with Art. 38 of the Fundamental Laws of Ukraine on the Body of Notaries, the consul completes a protocol of certification of faithfulness of a translation of a document from one language into another if the consul is fluent in the relevant language. This is done in case the translation has been carefully checked by consul against the original.

    Note: The Consulate General of Ukraine in New York doesn't provide translation services from one language to another.

    Forms of Certification of Faithfulness of the translators signature

    The consul certifies the authenticity of the translators signature on a translation if the translation of a document has been done by a translator,known to the consul, who holds the appropriate certification (certificate, diploma,accreditation) and who has certified the translation with his validating endorsement (affidavit).

    The Consulate accepts processing fees only in the form of money orders made outto the order of the Consulate General of Ukraine in New York.
    Cash, credit cards, personal or company checks are not accepted.


    Processing time

    The processing time for certification of a document translated is ten business days.

    IMPORTANT:
    1) The certification processing starts only after we have registered your documents.
    2) A drop-off day is not counted as a business day.
    3) If you apply by mail please allow one more day processing time.
    4) Any application received on Wednesdays, weekends, official Ukrainian and US holidayswill be registered by the Consulate only on the next business day.

    Note: There is no expedited service!

    If applying by mail, a return __self-addressed prepaid express envelope is required. When there is no enclosed such envelope, the documents will be kept at the Consulate, without charge, for personal pick-up, or returned by regular mail.

    = How to get Ukrainian visa?

    GENERAL PROVISIONS

    Passport and Visa required. Visas should be obtained from the Consular Office of theEmbassy of Ukraine in Washington, D.C. or Consulates General in New York and Chicago. No invitation letters are necessary for US, Canada, Japan, Switzerland, Slovakia and Turkeycitizens and citizens of the countries of the European Union for obtaining official,business, private and cultural and sports visas. For US citizens only: please make sure that your current medical insurance has coverage in Ukraine. No visas are required for citizens of Poland, Hungary, Romania, Mongolia, Yugoslavia (except Slovenia and Croatia)and the countries of the Commonwealth of Independent States (except Turkmenistan).Applicants holding passports issued by the following countries must allow at least 12 business days for their visa application to be processed: Angola, Afghanistan, Bangladesh,Vietnam, Ghana, India, Jordan, Iraq, Iran, Democratic Republic of Congo, China, Lebanon,Libya, Morocco, Nigeria, Pakistan, Syria, Somali, Sri-Lanka.Citizens of the above mentioned countries must additionally submit the following documents:

    Proof of readmissibility to USA (A copy of valid US Resident Alien Card, US TemporaryResident Card, employment authorization etc.)

    Original letter of invitation (No fax or e-mail copies) made through a Ukrainian" Visa and Registration Office" (For all types of visa).

    VISA TYPES

    Based on the purpose of entry, a BUSINESS, TOURIST, PRIVATE or a TRANSIT VISA is granted.

    IMPORTANT: Since only one type of visa is issued per entry, the primary purpose of entry should be stated in the application form.

    HOW, WHEN AND WHERE TO APPLY

    Each of the above visas may be applied for at the most convenient Ukrainian Consular Office on the visa application form. You can fill and print this form in your browser.(Forms are in PDF format. To read them you must install Acrobat Reader. You can download and print photocopies of visa application form (page 1, page 2, page 3, page 4).

    At the Consular Office applications are accepted in person only from 9:15 am till 12:00noon. For applicants' convenience documents may be submitted to the Consular Office by mail, a self-addressed prepaid envelope is required (for a return service please provide the Consular office with Express Mail envelopes only).

    Please be advised that the Consular Office does not accept any other kind of mail and does not bear responsibility for postal service. Any application received on Saturday,Sunday and official American or Ukrainian holidays (January 1 & 7, March 8, May 1-2& 9, June 6, June 28, August 24) will be processed starting from the next business day.

    TO OBTAIN A VISA AN APPLICANT MUST SUBMIT

    A valid passport, re-entry permit or refugee travel documents with at least one freepage for a visa. Copies are not acceptable.

    One completed application form (in Acrobat Reader) printed or written in block letters.

    One recent passport size photograph.

    If you submit a passport of a child alone it should be accompanied by a joint notarized written request from the parents (father and mother) or a copy of parent's visa.

    It is advisable to apply for a visa some three/four weeks prior to your prospective departure date. Due to the heavy fall of requests during the spring and summer months longer processing time (up to 12 days) may be required.

    A T T E N T I O N !

    No invitation letters are necessary for US, Canada, Japan, Switzerland, Slovakia and Turkey citizens and citizens of the countries of the European Union for obtaining official, business, private and cultural and sports visas.

    For other kind of visas ALONG WITH ALL THE ITEMS LISTED ABOVE THE FOLLOWING DOCUMENTS RELATED TO THE PURPOSE OF ENTRY MUST BE SUBMITTED (each hereinafter referred to as"the supporting documents") IN SUPPORT OF THE APPLICATION FOR:

    A) Business Visa: (except citizens of the above mentioned countries)

    An invitation (legible fax version is acceptable, scanned e-mail copies are not   acceptable) from a legal entity duly registered in Ukraine, typed on its letter head containing its official address and telephone number, signed by an authorized officer(other than the applicant) of the entity, bearing its seal and accompanied, when applicable, by a clear copy of its valid Certificate of Registration in Ukraine. The invitation must detail the purpose and if necessary the number of visits, exact travel dates, cities and/or towns to be visited and provide basic passport data of the invitee.

    B) Visa for Religious Mission:

    A letter of invitation from a Ukrainian religious organization for a foreign missionary must be submitted along with a written confirmation under the signature and seal of are sponsible official of the State Committee of Ukraine on Religious Matters or of the department on religious matters of a regional or city administration depending on where the sponsoring organization is registered.

    C) Visa for Humanitarian Mission:

    For individuals providing humanitarian aid, a visa is issued on the basis of a letterof support from the Committee on Humanitarian Aid of the Cabinet of Ministers of Ukraine.In case an applicant provides the consular office with a letter from the Committee onHumanitarian Aid of the Cabinet of Ministers of Ukraine, visa will be issued free of charge (application and visa fee are waived.)

    D) Tourist Visa:

    A letter of invitation from a Ukrainian or American tourist agency or confirmation from a hotel.

    E) Private Visa:

    for a home stay visit - only original invitation issued in the applicant's name by alocal Immigration Authorities of the Ministry of Internal Affairs of Ukraine where the sponsor permanently resides

    for medical treatment ; an official notification from the medical establishment providing treatment to the applicant

    applicants of Ukrainian background can obtain a P-2 type visa submitting the following documents:

    applicant's written request, copy of a Ukrainian birth certificate;

    money order for $100 made out to the Embassy of Ukraine;

    filled out application form;

    photo;

    valid passport;

    self-addressed prepaid envelope.

    Note: The applicants should apply in person and they are also subject to the interview.The Consular Office has full authority to evaluate and request more documents than submitted by the applicant.

    F) Employment Visa (including lecturing):

    A labor authorization document – (DOZVIL NA PRATSEVLASHTUVANNIA) in the name of the applicant issued by the Ministry of Labor of Ukraine and obtained for him/her in advance by the employer in Ukraine.

    IMPORTANT: Those wishing to enter Ukraine for PERMANENT RESIDENCE should call the Consulate for further clarification before submitting an application.

    G) Transit Visa:

    A valid visa to the country of destination or other documents which confirm entry intothat country or a confirmed one way or round trip ticket for travel from the point ofentry into Ukraine to the point of entry into the next country from Ukraine. This visadoes not authorize an applicant's stay in Ukraine and is not required for a non-stopflight over Ukraine.

    VISA DURATION

    Visa is valid from the date of issue.

    a) SINGLE or DOUBLE-ENTRY VISA

    For US, Canada, Japan, Switzerland, Slovakia and Turkey citizens and citizens ofEuropean Union countries - up to 6 months.

    For other nationalities and stateless persons - up to 3 months.

    b) MULTIPLE ENTRY VISA

    For US citizens - up to 5 years (the duration of visa can vary from 1 to 5 years).

    For non-US citizens and stateless persons - up to 1 year.

    Multiple entry visas can be issued in accordance with a decision of Consul General within 9 business days only.

    c) TRANSIT VISA  up to 5 days (a period of time sufficient to cross Ukraine bytrain, ship, automobile or other means of transportation).

    d) The duration of a P-2 type visa can vary from 1 to 5 years (by the decision of theConsul)

    VISA FEES

    Only Money orders made out to the Embassy of Ukraine in Washington, D.C. are acceptedby the Consular Office. Cash and checks are not accepted.

    Two money orders are required:

    (1) Application fee is $100* for 9-business day processing and $200      * for 3 - businessday processing

Visa fee is waived for:  exchange-students - US citizens (proper invitation required); children under 16years of age; If applying by mail, a self-addressed prepaid envelope is required (for a return service please provide the Consular office with Express Mail envelopes only).

    PLEASE BE ADVISED THAT THE CONSULAR OFFICE DOES NOT ACCEPT ANY OTHER KIND OF MAIL AND DOES NOT be completely responsible   FOR POSTAL SERVICE.

    ADJUSTMENT TO A VISA

    After a visa is received, it should be checked for accuracy and returned to theConsular Office, if necessary, for corrections. The Consular Office shall not be responsible for any discrepancies, which were not brought to its attention. If there is anecessity to adjust a visa, it may be done as long as it is permitted by the supporting document.

    VISA RELEASING

    When an application is submitted in person a numbered receipt is issued to a submitter.This receipt should be presented at the Consular Office when the visa is claimed. The travel document will be released to the submitter of the receipt unless its owner or other submitter has provided other written notarized instructions at the time of the pick up. If the receipt is lost, the travel document will be released to its holder or at the written notarized request of the holder to his/her appointee. If the travel document is to bemailed back, a prepaid, self-addressed return envelope with a courier service's air billis required from the submitter. The Consular Office is not responsible for lost, damaged,misdirected, or late - delivered mail.

     REGISTRATION IN UKRAINE

    Starting on July 1, 2001 no registration in Ukraineis required for aliens for the term of visa duration !! .
